What is the Community Cardiology Clinic
The aim of the Community Cardiology service is to provide a high quality service within a community setting. Its purpose is to triage, assess, diagnose and manage patients in need of non-invasive and non-urgent cardiac assessment and treatment.
The service is provided by a Cardiology Consultant and a GP with Special Interest in cardiology (GPwSI) with clinical and administrative support.
The service is provided in a General Practice setting on 107 Liverpool Road ,Birkdale; within The Family Surgery building.
- Triage of all referrals by a clinician to determine appropriate management.
- Face to face appointments only if triage deems it appropriate.
- Shared medical record ( where consent given)
- One-stop appointment for most routine appointments with short waiting time.
- Direct access to appropriate diagnostic tests.
- Timely communication of clinical management plan back to GP practice by writing and seamlessly into patient’s record.
