About the Role

People living in care homes should expect the same level of support as those living in their own homes. The national framework for the Enhanced Health in Care Homes (EHCH) model of care provides steps to achieve this. We are moving away from traditional reactive models of care delivery, to proactive care, that is centred on the needs of individual residents, their families and care home staff.

The purpose of this role is to have overall operational responsibility for the performance management of a multi-disciplinary, community-based care home team, working closely with the GP Clinical Lead. This team includes a variety of clinical and administrative staff and involves work alongside system partners to deliver the EHCH specification.

The Enhanced Health in Care Homes (EHCH) team is a forward-thinking team supported by the Southport and Formby federation. We are responsible for implementing the EHCH DES. We are passionate about working collaboratively with our Care Homes to enhance the care provision for those who live locally in their later lives. In my role as the EHCH GP Lead, I provide clinical support to all team members. We are hoping for skilled practitioners, who bring their own experience and personality, to join our wonderful team as we develop and continue to provide a truly holistic approach for our Care Home residents. -Dr B Pennington

Main duties of the job

We wish to recruit an Advanced Clinical Practitioner with advanced skills to be responsible for the day-to-day management of the clinical team including a trainee ANP, Specialist Frailty Nurses, and Nursing Associate, with support from the Advanced Pharmacist Practitioner, GP Clinical lead for the service and the Human Resources Manager.

The clinical role will require the post holder to have extensive experience in assessing the needs of older people, including experience of the holistic comprehensive geriatric assessment and be able to work independently within the Care Home environment. The post holder will require advanced communication skills to discuss and complete proactive individualised clinical management plans for Care Home residents. The desired outcomes from assessments include improving the health experience of Care Home residents, having a positive impact on primary care service provision, improving interprofessional relationships, and reducing unnecessary and potentially harmful hospital admissions.

Although the post is advertised as 30 hours part-time, we will happily accept applications from those individuals who are looking for full-time hours.
