About the Role

Southport and Formby Health (SAFH) are a GP owned company created to enhance the delivery of healthcare services to people living in Southport and Formby.

Southport and Formby Health work collaboratively with Southport and Formby Primary Care Network (PCN). Southport and Formby Primary Care Network is a group of 15 GP Practices located in the Southport and Formby area who work together with other local organisations to improve the health and health care for our local population of around 129,000 people.

The role will support the Digital Clinical Hub at Southport and Formby Primary Care Network. The post will help to support GP practices remotely by performing key core functions such as:

The individual must also be willing to undertake the appropriate training as set out by the Personalised Care Institute as part of the national framework for Care Coordinators. The individual must have experience of using clinical systems such as EMIS or be willing to undertake the necessary training, this is essential to the role.

This role is a new role within the PCN, and advertised as full-time, however part-time applications will be considered.
