About the Role

The main duties include but are not restricted to;

The post holder will work alongside Care Co-ordinators to coordinate and communicate individual care plans with system partners and develop relationships with care homes, community teams, mental health services, palliative care services, social services, and secondary care frailty teams, to enable residents to meet their health goals and be cared for in their preferred place of care.

You will follow standard operating procedures within the limits of your defined clinical competence, and this will be reviewed regularly as the role evolves. You will work within your team and have the opportunity to escalate concerns and complexities to the multi-disciplinary team meetings.

The role will also include working with the Primary Care Network (PCN) to identify gaps within local service provision and work to reduce health inequalities within the learning disability provision. This may include supporting GP practices within the PCN to complete annual health checks for the learning disability population who do not reside within care homes.

As the role evolves it will lead to education sessions in a group setting exploring health education, screening processes and conditions within the learning disability population.

The role is a fixed term contract ending on 31.03.2025 and although advertised as full-time, part-time applications will be considered.
